We accept referrals from care coordinators, social workers, ward staff, commissioners, families and self-referrals. Every referral is acknowledged the same working day.

Same day
We acknowledge your referral and confirm who is picking it up.
Within 48 hours
In-person assessment on the ward or in the community.
Within 3 working days
Written support proposal and weekly costing issued.
Agreed date
Move-in with a personalised care plan in place from day one.
Funding
How placements are paid for
We work with local authority and NHS commissioned funding, spot purchase, joint funding, Section 117 aftercare — and we accept direct payments made through an intermediary.
- Direct payments accepted via an intermediary, managed account or third-party payroll provider
- Personal health budgets and personal budgets held on someone's behalf by a family member or broker
- Local authority and NHS spot purchase, block and joint-funded packages
- Section 117 aftercare funding, including split funding between health and social care
- Written costing with hours and staffing issued within three working days of assessment
- Clear, itemised invoicing — no hidden charges added after a placement starts
Continuity of care
Small, named staff teams per house and minimal agency use. The same faces week after week, with the same manager and the same clinical contacts — that consistency is what keeps placements standing.
Governed by CQC standards
Uplift Care Group Ltd is a CQC-registered provider and works to the fundamental standards: safe care, safeguarding, dignity, staffing, and good governance — inspected, not self-declared.
Audited, recorded, evidenced
Digital care records, medication audits, incident review, supervision and property compliance checks — so commissioners can see exactly what was delivered and when.
